Find out morePre-admission is an important part of your day hospital care. If you have any questions about your planned day surgery admission, the completion of forms, cost or your health insurance status, please contact our team who will be happy to assist you.
As part of our ongoing response to managing COVID-19, please be aware the following will occur on arriving at the hospital.
To ensure we can confirm your admission, health insurance and other details/arrangements please complete our online preadmission form at least 5 days before your admission.

Alternatively, you can download and print the IVF and General Surgery assessment forms and fax them to (07) 4977 5199 or email as a PDF to admin@msdh.com.au
Please notify us if your weight is over 150kg prior to your admission as we will need to organise a pre-admission assessment.
If you have private health insurance please contact your health fund to check your level of cover for hospital admission.
Please ask these five questions:
After a general anaesthetic, for your safety, you must have an escort to accompany you home, and a responsible adult must stay with you overnight. If these cannot be organised, please contact the day hospital at least two days prior to your admission to discuss your discharge plan with one of our registered nurses. We can assess your scenario and liaise with your specialist/anaesthetist and they can assist with discharge planning. If you do not have an escort, or overnight carer, then your procedure may be cancelled.
Arrange two escorts. If driving home, you must arrange for a child under 10 years old to have two responsible people present in the vehicle (a driver and a carer).
If you can’t speak or understand English well, and do not have someone who can speak English to accompany you to the hospital, then please inform our team before your admission date so we can arrange an interpreter.
If your procedure is subject to WorkCover or a Third Party Claim, please send full details of the claim to the hospital, including a letter from your insurance company accepting liability for your admission to hospital for the procedure.
If you are having a general anaesthetic or IV Sedation, do not eat any food for six hours prior to surgery. You may drink water up to two hours prior to surgery.
Avoid drinking alcohol or smoking for 24 hours prior to surgery, as this may affect your recovery.
You will need to contact Mackay Specialist Day Hospital one working day prior to your surgery to obtain details regarding your admission, such as admission and fasting times, fees and other relevant information. If the nursing staff require any further information regarding your medical or surgical history they may contact you prior to your surgery.
